PATIENT INSTRUCTIONS

Become an active, involved and informed member of your own healthcare team. We encourage you to voice any questions or concerns, pay attention to the care you are receiving, educate yourself about your condition – including the medications you are taking – and participate in all decisions about your treatment.

Surgical Patients

Please arrive one hour before your appointment. Your physician’s office will let you know if there are specific pre-surgery instructions. Please understand not following these instructions may require us to reschedule your appointment.

Lab/Imaging Patients

Please arrive 15 minutes before your appointment. Your physician’s office will let you know if there are specific preparatory or dietary instructions to follow. Please understand not following these instructions may require us to reschedule your appointment.

Walk-In Imaging Patients

Walk-ins are welcome, but pre-certification may be required. Your physician’s office will let you know if there are specific instructions. Please understand not following these instructions may require us to reschedule your appointment.

REGISTRATION

Prepping for arrival

When you arrive, we will need a photo ID and your most current insurance, as well as a current copy of any secondary insurance you may have.

Our business office will then verify your insurance carrier(s) for hospital services.

To ensure that you receive maximum reimbursement from your insurance carrier(s), please note whether you or your physician need to meet any admission requirements, such as pre-authorization, physician referral or second opinion. Failure to meet these requirements could reduce or even eliminate your insurance benefits.
